EULENSPIEGEL PUPPET THEATRE

In support of the Creston Arts Summer Art Camps, Crest Area Theatre is sponsoring a residency by the Eulenspiegel Puppet Theatre.  Puppetry is an imaginative melding of creative visual arts and theatre found in cultures all over the world going back millenia.  This is the 50th anniversary year of the Eulenspiegel Puppet Theatre, a non-profit organization based in West Liberty, Iowa, dedicated to promoting the art of puppetry through high quality performances, workshops, and creative events.  The company is committed to educating the public about the arts, serving as a center for the exchange of ideas, and enhancing the quality of community life. 

Eulenspiegel Puppet Theatre will provide morning and afternoon workshops from June 24th through 28th. The morning workshop will be based at The Creston Arts Center for 20 students in grades 2nd through 5th, creating and exploring the use of different forms of puppets. The afternoon workshop will be based at the SWCC-PAC Auditorium, for 12 students in grades 6th through 8th.  Participants will explore rod-puppet making, as well as rehearse and produce the puppet show "Remembering Buxton".  Students in the afternoon workshop must also be available for the July 28 Friday evening performance.

I give my child permission to participate in Eulenspiegal Puppet Camp. Therefore, in exchange for Creston Arts/Crest Area Theatre allowing my child to participate in Eulenspiegal Puppet Camp activities, I understand and expressly acknowledge that I release Creston Arts/Crest Area Theatre (including Creston Arts/Crest Area Theatre Camp staff, instructors, and volunteer mentors) from all liability for any injury, loss or damage connected in any way whatsoever to participation in Eulenspiegal Puppet Camp activities. I understand that this release includes any claims based on negligence, action, or inaction of the parties listed above. I have read and am voluntarily signing this authorization and release. I hereby grant permission for my child to participate in all activities provided by Eulenspiegal Puppet Camp.​

 

Authorization for emergency medical treatment:

 

If my child should become ill or injured during Eulenspiegal Puppet Camp activities, I understand that Creston Arts/Crest Area Theatre will I) contact me immediately or 2) contact the person(s) I have designated in case I cannot be reached. Should Creston Arts/Crest Area Theatre staff be unable to reach me or the person(s) I have designated, Creston Arts/Crest Area Theatre staff are authorized to contact my physician or arrange for immediate medical treatment to ensure the health and safety of my child. I accept responsibility for the payment of medical services provided.

 

Photo/Video Release:

 

I grant permission to Creston Arts/Crest Area Theatre to use photographs and videotapes taken of my child for Creston Arts/Crest Area Theatre publication and promotional purposes.
